5th week : EAC Chase Farm


WARNING : Long entry, lots of medical jargon.

3 days at Emergency assessment centre / acute assessment centre, Chase Farm. this is an emergency mental health centre who offers assessment on possible mentally ill patient, new or relapse case. Operates 24/7.


Monday&Tuesday:
Bank holiday, ataupun cuti umum, bukan cuti untuk org yang kerja bank sahaja.



Wednesday:
first day at EAC. gugup dan gagap. tapi orang disini seperti biasa, super nice. I met Mohamad, who was a charge nurse there and he introduce me to other staff, sheeba who is another nurse, Dr Black the Psychiatrist, ( his a white guy anyway), Rob (Admin worker), and the receptionist whom I forgot her name. aiya! I was expected to learn more about how this unit function, their routine and liaison with other unit and they would like me to participate with assessment on patient, of course with a senior nurse assessor. oh ya, here, nurses here are called nurse assessor because basically, they do assessment on patient.

what I learned?

- the flow of case referral. there were registration (for first contact patient), fill in form and rio, record in referral book, assessment, triaging of case. triaging is where they decide whether the patient is really acute or not.
    * more of social problems and need support system : reffered to service line e.g. Bureau of drug alcohol
                                                                                    service, or other social services.
    * Not acute, able to be treated in community : Primary Community Mental Health treatment (PCMHT)
    * Acute : will be assessed again with members of HTT to see if patient need ward admission or can be
                   seen at home basis. HTT is basically a gate keeper to ward admission.

achievements:
- observed mental health assessment
- written patient observation and assessment outcome on RIO.
- Written the risk assessment on RIO.


Thursday:
was a productive day. I was introduced to Judy and Neela, whom both are nurses.

learning:

1.  Mental health act.

- how patient are to be sectioned in different2 setting.
- there are one clause that lined the nurses authority to detained pt in ward if he/she wished to abscond. for Psychologist, only those approved by sectioned 12 (which they need to sit for exam) are allowed to section patient.
- The act can be used with one another. and if there is a need to detained pt for a longer period, sectioned must be renewed before it expires.
- patient are sectioned because they refuse assessment and treatment eventhough they are at risk of harming self and others. they are detained for further assessment in wards and/or treatment.
- there are other section for forensic cases where i don't really read in depth, nice to know je, kat malaysia pakai law lain.
-Patient dan carer sangat tahu tentang hak mereka kat sini.

2. Assessment at ward setting with Psychiatrist. sama jugak dengan assessment kat EAC cuma lain sikit approach.

3. Assessment patient yang ade personality disorder with Kwadwo (nurse). haish, penat dengar orang yang putar belit cerita ni. Tapi menarik jugak la. kene pandai assess betul2. next, assessment on suicidal patient, who intoxicate self with pills and alcohol.

Achievements:
- Neela help me a lot today. I was able to go through all the documentation from scratch!
- jejak kaki kat wad medikal. ya, macam wad hukm. baju nurse pun warna biru jugak. tapi patient sini pakai baju sendiri, xde uniform. doktor pun x pakai da white coat. infection control maybe...


Friday :

Mohamad was in today and he asked me to start the assessment of one of the patient. arghh..ketaq lutut tapi excited! tunggu punya tunggu sekali patient tu abscond dari A+E. ape da nasib. Had lunch with Neela today, she tells stories about her hometown mauritius. turns out that Mauritiius sound familiar with Malaysia, in terms of its diverse ethnicity and religions, like she is christian and Mohamad is Muslim. kat sana pun ada orang cina dengan india, melayu tak sure. also learned that UK used to import a lot of nurses from Mauritius, patut la ramai nurse dari mauritius kat sini.

learning :
- had chance to communicate with patient during assessment, asked few questions. though is not a full assessment, tapi ade la something. huhu.

- decided to stay back late. sedih tak dapat tengok mental health act section 136. last week banyak case, minggu ni xde pulak. ape yang best sangat section 136 tu?? sebab meriah. tengok patient di escort oleh polis, ade ambulance, ade keta polis. tapi yang penting nak tengok macam mana law itu di aplikasikan. yang lain2 tu entertainment je. section 136 ialah kuasa yang ade pada polis untuk menahan siapa2 yang membuat kacau di tempat awam, yang mana siapa2 itu mempunyai contact dengan mental health service. bahasa inggerisnya: power held by police to detained a well known mental health patient who caused ruckus  or imposed threat to community or to self in public area. Lasted for 72 hours, so pt need to be reviewed by psychiatrist within that time, to decide whether he/she need to be put under other section or not.

achievement:
-x dapat tengok 136, section 2 pun jadi la. section 2 Mental heath Act ialah right to detain patient for 28 days for assessment in wards. meriah la, ade polis bawak patient in handcuff. woo, rupanya dari pukul 1tgh hari patient tu da kene tahan dalam ambulance dengan polis, pukul 7 baru sampai hospital. Polis yang bawak patient tu pun semua baik2 belaka, ade nak dekat 6 orang jugak la.


Masuk ward untuk pinjam kerusi roda. staff sana garang. Diorang tanya kenapa tak ada tag. I said I was temporary student, but they were still unsatisfied which is sooooooo obvious in their expression. well, though they were right to inquire identification from me, they could ask it in more 'friendlier' manner. dah la noob gler td nak bukak pintu pun tak reti. adeh malu nye.. huuu, takut plak nak posting kat ward.


Overall, best posting kat EAC sebab tak tahu kes apa yang akan datang. ade yang manipulative, ade yang betul2 aggressive, ada yang sihat wal afiat je, ada yang betul2 disturb dengan suara. yeah I like it here. staff sporting, siap buat appoinment lagi nak ambik gambar sebelum balik malaysia. hmm.. sayangnya 3 hari je ade kat sini. Dah kenal dengan ramai staff, makin berat hati nak tinggalkan Chase Farm hospital.....

Boxing day

assalamualaikum


It's the days after christmas that excite me the most. Boxing day! Mula-mula ingatkan ape la boxing day tu, rupanya hari orang keluar shopping. Memang macam nak boxing pun. Jalan sikit kene tolak, kene langgar beg org, bakul orang, stroller baby. hadeh. Tapi serious nak pengsan tengok harga diskaun barang-barang kat sini. (simptom penyakit shopaholic datang balik). Memang gila la, sampai 70% untuk barang2 branded. Selfridges, Adidas, Swarovski, ZARA, GUESS, Hamleys, huh, crowded sampai kene que untuk masuk.


Rasanya orang kat London shopping setahun sekali je kot, sebab beg shopping masing2 penuh melimpah ruah dengan barang. Selain tu pelancong dari merata pelosok dunia pun ade datang shopping. Orang Malaysia tak yah cakap la, toleh je muka orang melayu, adoi dah rasa macam kat Malaysia pulak.


by the way...this is Oxford Street, heaven for shopaholic. Branded stores to economic supermarkets, souvenirs stall, just name it and you'll find it here.


Tengok price tag dalam harga pound rasa bahagia je hati nak membeli, tapi bila convert RM, mau muntah darah dibuatnya.. Tapi kalau orang yang memang minat designers brand, tak rugi beli sebab beza harga ratus jugak la dengan harga kat Malaysia. Apakan daya, tak mampu jugak nak beli. tunggu la ade gaji sendiri.


So masuk la kedai yang kurang sikit jual designers brand. contohnya Primark. Dengan 5GBP pun bley dapat at least sehelai top yang cantik. but there's a catch, look properly at the goods you've chosen. don't just buy for quantity, mind the quality. after all you re spending in GBP. and yeah, as usual, disebabkan melaut manusia yang datang shopping kat sini, traffic pun jammed.


waa..bahagia nya. My 'Shopaholic' dream to shop in street of London dah tercapai. I spent the most compared to Gervanne and Joanne but still within my budget. *sigh...I rarely shop with list. Just do it on impulse. people label it as freestyle shopping. ada pulak macam tu. Most of shops here provide this big cloth bag for shopper to stuffed in all the goods that catches their eyes. I never took any of those cause I know I'll end up filling them up unconsciously. pegang kat tangan je la. nanti da lenguh tu maksudnye da terlebey suda ambil barang. so this is another tips to avoid overspending in UK. 
Do NOT TAKE THEIR BIG CLOTH BAG!!! 


Whatever it is, I'm satisfied with my 'catch' today. No regrets!

Merry in a Hurry

assalamualaikum...

I'd heard about this 'Secret Santa' game, discussed among the members of HTT since the end of November.  I personally know what this game is all about and decided that I don't really want to join them. I don't celebrate Chrismast but I do respect their celebration and enjoyed the holidays given. Anyhow, I thought they'll do it somewhere else. Yesterday, the day had finally come. Turns out that the exchanging gift process was done at our very own small office. (huh, 'our' konon!).

By the way, Secret Santa is all about exchanging gift for Christmas. Present were given anonymously within a group. They celebrate in in humble and simple way. 

Italian cake and crackers. satu pun aku tak rasa...tak tahu asal usul~~

Mungkin...
                 ....doa yang terjawab itu bukan semata-mata kurnia daripada Allah kepada hambaNya yang
                     berusaha.

Tapi.....

                 ....tak lebih sekadar ujian samada hambaNya itu sungguh benar taat atau bagai lalang yang sering
                     leka dibuai haluan angin. Mudah berubah.


Rasa sakit.

                ....bila diperingatkan bahawa diri ini telah leka dalam rahmatnya. Sakit fizikal yang mungkin
                    sekadar kecuaian diri. Barangkali ada hikmah tersembunyi dibalik perkara yang terjadi.

 
Rasa takut...

Kerana....
 
                ....seperti gagal jadi hambaNya yang kekal taat bila ada ujian yg diberi dibalik rahmatNya. Perlu  
                    muhasabah diri. Perlu lebih kuat lagi. Semoga aku tidak leka dan Allah tak jatuh murka.

3rd Week is a...urm....

assalamualaikum..

It's already third week being here, in HTT Chase Farm Hospital. This is not my productive week. Been following few home visit and then spent the rest of the day at the office, reading books available at their bookself, which I read twice..most of them. hmm...everyone was just busy with their work, each wearing tense expression. but they are super nice people. They will still responds to me if I had anything to enquire. It's just, me, not really want to bother them when they are busy. So there I left, sitting for hours, reading through my case study.

On Monday, I was supposed to go home visit with CJ, a nurse there but turns out that most of his client today had tendency to be aggressive. So he just apologized and said I can't come. When he came back, he greeted me with surprised expression, asking

 'have you been sitting here all day long??' 

i answered,

 'nope, I went to the loo twice.'

and then he laughed, again said sorry for not being able to include me in his visit. He was the only nurse on duty in the team that day, the other one was a senior nurse-cum-team leader that happened to be so busy that day that I only catch his glimpse.


On wednesday, I woke up with massive headache and feverlike symptoms. I can't even stand straight. it's not migrain cause I don't experience photophobic. turns out it was PMS. ceh.. I inform them that I can't come and there goes a day wasted.


Today was not much different, but at least I was included in face to face Transfer of Care for a patient. The transfer of care marks the termination of care of patient by HTT and the role is resume by the patient's care coordinator. Care coordinator will be the one who will be incharge of the patient while they are in community setting. they can be doctor, community psychiatric nurse (CPN), or social worker. There was ECT session going on, but there's no staff from HTT are available to attend so there goes my chance. hmmm...



This is the downside of being an adult learner. You need to be independent yet an opportunist. Muka kena tebal bak kata orang melayu. I should have been bold and just ask any psychiatrist to join their assessment. Hmm...No time to laze around.


and..finally! Snow falls today! On my way to the hospital, it was raining and bit by bit, the rain drops start to crystalize and lastly, it formed a whitish soft ice drop callled SNOW! I was grinning excitedly walking under the snow while others were protecting themselves with umbrella. haha, what a way to end this gloomy week!


....it had not snowed heavy enough to this extent...

'Bloody' Monday

Assalamualaikum...


It was 6.30am in the morning. there were no signs that the sun is rising. It was cold. too cold. I know that i'm late, but i'm too lazy to move. so I walk like a zombie, tip toed down the stairs, not wanting to wake other tenants from their sleep. I brushed my teeth with eyes half open. then, I took anything that I can reach in the closet to dress up. perform subuh prayers (yes it was late..).  Joanne and Gervanne were still in their la la land. I need to be early to catch two buses. or else I will be late to Chase Farm Mental Health Unit. Even then, I was still yawning and move like a snail. I slowly put on my hijab when suddenly a hot slimmy sensation was felt near my left nostrils. I took a glance at Gervanne's small rectangular pink coloured frame mirror and say..."oh it's blood.......



.....after few seconds.....




WHAAAT, IT'S BLOOD!!! OOZING BLOOD!!"



quickly, I applied the knowledge that I shared in class presentation during my first year. how to manage nose bleed (epistaxis). my index finger and thumb were used to applied soft pressure on my nose and tilt up my head slightly to stop bleeding. then i realize, hey, this is my first ever experience of epistaxis! past few days were just blood flakes that clogged up my nose. hmm, not something to be excited off.


but at least now I am 100% awake. and yeah, I'm 5 minutes delayed from my usual 'departure' timing to the hospital. yikes!

Kebab

assalamualaikum..

AMARAN: Tidak sesuai dibaca oleh mereka yang tengah lapar dan xde duit nak beli makanan.



Nak cerita pasal makanan lagi. sape suka makan kebab?? 


ya muzaffar muhaimin suka! my younger bro yang selalu cari kebab tiap2 kali pergi pasar malam. kat area north london ni memang banyak kedai jual kebab. kalau satu deret kedai tu ada 10 kedai, 4 daripada kedai tersebut adalah restoran yang jual kebab. macam2 nama ade. jadi sebab da selalu sangat tengok lama2 teringin pulak nak cuba. hmmm...mesti bahagia kalau ko duduk sini kan Mimin.


anyway, the place that catches our eye and stomach is called Sunshine Kebabs. it sounds weird with S at the back. it is right near to Primark (shopping place) in Wood Green.

apa yang ade kat situ? 

they sells kebab la, but you can have it in form of burger, wraps or platte. you can either have chicken kebab or lamb kebab. just like any other fast food restaurant, you can order them in meal set with fries and drinks. selain tu salad, cheese burger ngan grilled chicken pun ade jugak.

fries yang besar-besar gedabak..cukup rasa kentang, memang sesuai la untuk orang yang gila kentang..

sangat-sangat sedap. thumbs up! saing-saing dengan GCB kat McD la. i ordered chicken kebab burger with fries and drinks while Joanne had cheese burger. finger lickin' good!! 

tengok ayam dia melimpah ruah..lengkap la rasa dia. ada salad, tomato, bawang, cheese dan of course isi ayam or kambing, pilih mane korang suka. diameter, lebih besar daripada zinger burger KFC. yang paket biru kat atas sikit tu mayonaise. nyum..NYUM...



hmm..burp..waiter bill please!! 

harga agak affordable. mine was 3.99 pound. Joanne's 99 pence. yes, it is cheap in pound. but if you convert it, hmm, that's another case. (darab 5, gi congak sendiri..). Yang penting ade cop halal! yeah, support muslims' product!  



bad news utk kawan2 kat Malaysia, diorg belum buka cawangan lagi kat sana. paling dekat yang korg boleh pergi kat Australia. haha, pe punya cadangan la, pergi Australia semata-mata nak makan kebab. anyway, siapa2 yang nak buat bisnes, kalo tak salah ada peluang untuk bukak franchise. just bukak je website kat bawah ni haa...

di sempadan halal dan haram

assalamualaikum..

menjaga batasan halal dan haram. hingga kini masih kurang selera makan..ada pilihan tapi tiada selera..

sebenarnya agak terkejut sebab ramai orang yang telah ditemui diutara london ini tidak mengetahui tentang makanan halal. rasanya, ramai juga saudara seagama di bumi UK ini, apatah lagi di kawasan menetap sekarang. agak susah juga memahamkan mereka kerana sikap mereka yang terlalu mahu tahu hingga tak tahu macam mana nak bagi tahu mereka. bagi mereka, semua restoran timur tengah itu halal, tapi pada mata ini, jika tiada tanda halal maka tidak halal la makanan yang disediakan disitu. malah, hanya untuk membeli roti sahaja pun perlu bagi diri ini untuk membaca setiap butir perkataan yang tertulis di bahagian 'ingredient'. hanya yang berasakan tumbuhan dan bebas alkohol sahaja yang dipilih. memang..memang diri ini cerewet. kerana jika ragu-ragu, bukan sahaja jatuh hukum syubhah, malah anak tekak juga rasa kembang, tak sanggup menelan.


dilema juga dalam menjaga hablum minannas dan habluminAllah.

perit. perit bila mereka mempelawa makan bersama, dan dengan rendah diri terpaksa menolak. terasa agak kurang menghormati orang lain disitu, tapi apakan daya, diri ini lebih menghormati tuntutan agama. sedaya upaya pelawaan itu ditolak dengan berhemah. mujur mereka memahami. pernah, sedang leka membaca nota pesakit, seorang rakan sekerja mempelawa makan jeli. tanpa sedar, tangan terus mencapai dan disuap ke mulut. tidak semena-mena, tekak terus memberontak, lantas dengan cepat berlalu ke tandas dan memuntahkannya kembali. rupanya terlupa, bukankah majoriti jeli disini mengandungi gelatin, dari sumber yang sudah pastinya haram. patutlah rasa pun semacam. terima kasih tekak kerana mengingatkan otak yang mungkin sudah lesu. tidak tahu apa hukumnya, yang pasti, Allah tahu sungguh aku tak sengaja. dan Allah itu maha pengampun.

jujur, sangat menghargai keprihatinan mereka dalam memastikan diri ini tidak kebulur di musim sejuk yang dinginnya makin menyeksa hingga ke tulang keting. mujur, tempat mastatutin sementara ini berjiran dengan kawasan Islam. Ya. kawasan Tottenham, yang mana hijab dan tanda halal di kedai2 tidak lekang dimata. ya, sekurang-kurangnya Allah telah tunjukkan jalan, jadi, pandai2 lah nak hidup.



hanya air masak untuk mengisi perut...dan menjaga hubungan sesama manusia..


ps: Vapianno adalah restoran Itali yang berkosepkan dapur (kitchen concept). bila anda masuk, pekerja mereka akan beri satu kad ala2 kad atm, yang akan merekod order yang dibuat. carnya, masa order, beri kad pada kitchen crew dan mereka akan swap, ala2 macam swap kad kredit bila overbudget masa shopping tu. lepas tu, kalau pesanan yang memerlukan pelanggan menunggu, mereka akan beri satu alat beeper. bila makanan siap beeper tersebut akan berbunyi 'beep..beep..action kamen!' oops, ok, xdelah, bunyi vibrate je. lepas tu pegi lah collect makanan kat kaunter. and yeah kalo korg bace betul2 ada satu benda yang reatoran ni xde, hah korang perasan x? cepat teka-teka...

table presentation..bukan arak, cuma garam, pepper, cili, minyak zaitun dan yang itam tu x tau..

ya, disini xde waiter pun, yang ade tukang jaga kaunter pembayaran dengan tukang kemas pinggan je. masa order pun, direct dekat chef yang akan masak makanan tersebut. jimat kos kan??!! anyway, restoran ni x halal, just nak kongsi je sebab suka konsep dia. kalau kat Mak yong pun buat konsep mcm ni kan bagus, x payah tunggu lama, tunggu beeper vibrate je pastu terus ambik makanan.


p.s.s: relative gervanne yang belanja dinner kat sini, kalau tak, tak jejak kaki pun..tak mahu dan tak mampu..

home treatment team

assalamualaikum...

it's been three days, at Home Treatment Team, mental health unit, Chase Farm Hospital. So far, things had been interesting. I've involve in home visit with different line of profession, psychologist, nurses, and social workers. before you get more confuse, let me brief a bit on how their mental health system works.

nursing in UK were trained according to 5 areas of specialties: general nursing, peadiatric nursing, maternity nursing, learning disability nursing and lastly, mental health nursing. Every nurses who specialiazed in mental health nursing will soon join the mental health services, governed by National Health Services (NHS). NHS is like KKM in Malaysia.

ok, Mental health services here rarely admit patient in wards, since they had abolished asylums and due to limited number of beds in wards. their system promotes treatment of mentally ill patient in community based. To   do so, teams such as home treatment, primary care treatment and community mental health treatment was set up. I'm still not clear on how different2 unit function, but this is basically how things works there.

the one i'm attached to currently is called home treatment team (HTT). this team is considered as front liner and operate 24/7 and staff works in shift. whenever a patient is sent to mental health unit emergency, regardless new admission or case of relapse, they are considered as 'acutely ill'. HTT's members will perform assessment to decide whether that patient needed to be hospitalised or can be treated at home. when they decide NOT to hospitalised that patient, then their roles begin.

they will start by assessing the patient at home or at hospital or primary care center, and determine plan of care (i.e: identification of problem, frequency of visit, medication, social needs and etc.). Their intervention only applies during acute phase, and ended when patient was able to engage with reality and comply with treatment, usually for 4 to 6 weeks. then, the HTT responsibility terminates when patient is assigned under care coordinator, under their respective GP.  erm..still not clear whether GP chose the care coordinator or vice versa.

anyhow, still got time to learn...till then~~~
